- the address from which the correspondence is forwarded;
- the address to which correspondence is forwarded;
- the time period within which the transfer must be ensured (minimum time period for the provision of the service – 1 month);
- in case of authorisation, a power of attorney shall be submitted, which certifies the position to be held and the right of representation of the person, presenting a personal identification document.
